As an aside, Louisiana does many things poorly, but we led the nation in getting actual reconstruction surgery fees to ve covered under your insurance plan.
Insurance companies fought this as they considered this to be "elective cosmetic" surgery.
Last time I checked, no one elects to get cancer, not to mention the emotional toll it takes on women.
Now, of course, those surgeries are covered, and other states have copied our laws.

This is no different than any other covered procedure. If you go to a provider that is not in network with your plan, your benefits are lower and you are responsible for paying the amount over what your plan allows.
