Yes. Poverty in and of itself is a disease. It leads to poor access to care and less ability to afford proper care.
Worse, though, is lack of education. Oddly enough, lower education corresponds with more utilization of healthcare resources (ER visits for non emergencies, hospitalizations for poor adherence to diet and/or medications).
Add to that the highest rate of smokers is in poor, uneducated people, and you've added fuel and a spark to the kindling.
Poor education leads to poor families who frequently live in less desirable, older houses nearer major highways and inner cities which lead to higher lead levels which dull IQs. Toss in food insecurity/malnourishment of the young, developing mind, and you've got a cycle that's difficult to break. That same population is at a higher risk of essentially every lifestyle-influenced disease (obesity, hypertension, diabetes, high cholesterol) which lead to all kinds of secondary health problems because they're less likely at each step:
1) having access to a primary care doctor (and having health insurance isn't the same thing as having access to care)
2) affording medicines (for the diseases listed, a number of fantastic medications can be had for $4/mo, but...)
3) less likely to adhere to regimen (they don't understand why the medicines are important because they don't make them feel immediately better when they take them. They don't understand what a healthy diet is or how food can hurt them. I've read (and believe) that the average Louisiana diet contains 12g of sodium/day (recommended is ~20% of that)).

Most people in rural areas that live below the poverty line smoke and/or use smokeless tobacco... Smoking in particular is the cause of many of their health problems

Well frick. Somebody should start protests against Whole Foods to get them to locate in the poor neighborhoods. Where are the SJW's when you need them?

a lot of the poor folk I grew up with have medical problems stemming from using doctor prescribed pills their whole lives.
They would go to the doctor for some ailment like a back spasm and get a pill, which causes this side effect for which they got another pill, which caused them to lose sleep which they got another pill, which raised the blood pressure or other levels to rise for which they get another pill
Pretty soon they are always down and out because their immune system is shot so they have some kind of whopping cough and also retaining fluid in their legs or 1,000 other things.
Most of these folks don't get a problem actually treated because that takes insurance and money. So doctors just throw medications at it and a lot of these people become dependent on multiple different prescriptions, so this along with the chain smoking/drinking/eating causes a lot of poor health.
course sometimes folks are just white trash and health problems could stem from mold in the single wide or smoking some bath tub meth...'Merica!
