I was local tv news producer 20 plus years ago. When a national story is fed to affiliates it comes with a suggested intro. The proper thing to do is to either modify the suggestion or write your own, but due to time crunch or laziness, they will often just cut and paste.
As with most things, what is called conspiracy or collusion is most often laziness.

Thanks for the clarification but I would suggest it’s an integrity issue as well. Particularly when it is a more important story or a political story. Then it is just propaganda, even if the actual reader doesn’t realize it.
