Sounds as though he is near/at end-stage COPD.
Is what it is because any little thing a cold/allergies/whatever etc can cause an exacerbation , and the maintenance drugs can only do so much.
Is he on a maintenance antibiotic?
Unfortunately, this pattern will continue until he either dies from an exacerbation, or chooses hospice.

We waste an exuberant amount of resources focusing on quantity instead of quality of life. Have seen several people be declared essentially brain dead, but family prays for a miracle and leaves them on life support for a month burning through resources.
Same with folks with ESRD that are non compliant with dialysis or medications and end up with hospitalizations every week from fluid build up.
