After reading through the thread more clearly this time, it is apparent to me that a lot of people don't know the difference between a straight steal and a hit and run.

Either 1 of the 2 of them missed the sign, or Ross recognized the obvious pitchout being 4 feet up and 2 feet out and decided not to waste a swing, which is a baseball no-no of course, but still seems the most likely IMO.
I highly doubt Mainieri called a run and hit on an 0-0 count and I very much doubt he called an Ibarra straight steal. So either Ibarra had the green light to run if he thought he had a good jump (also very doubtful) or Mainieri called a hit and run.
