After reading both, the symptoms largely resemble Take All Root Rot. especially the part about easy to remove stolons. I'll start with disease X to try and get it under control. some of the areas seem already beyond repair. what would be the best practice for re-sodding? i dont want to re-sod just to have it occur again.

Being in NO you could likely re-sod now, but it might behoove you put annual or perennial ryegrass in those dead areas now to grow during winter and re-sod next spring as needed in the larger dead areas. Good thing about St Aug it does fill in pretty quickly during active growth. However, you do want treat the fungal disease now.

If ronk re-enters the thread he can better give you suggestions on treating take all patch. In addition to the fungicide treatment with Scott’s Disease Ex (Azoxystrobin) he will advocate applying peat moss, which b/c of it low pH has shown help control the fungal disease. You’ll might want rotate fungicide treatment, following azoxystrobin, with propiconazole fungicide, to reduce fungicide resistance. Follow label directions on the fungicides with regards to application rates and re-treatment intervals.

Here is another article on take all patch from Texas A&M that provides a little more detail on treatment. LINK.
