I've had them since I was 10, I'm 52 now. For me, finding a real neurologist was key (I'm in MS, don't laugh). We've finally hit on a great mix of preventative drugs and rescue drugs when needed. If you're working with a neurologist, I'm sure you've tried everything under the sun to reduce your migraine days. I just switched to the new Qulipta and it's great so far. What does your doc propose about the temporary blindness?
Fortunately I don't have any eyesight issues. My weirdest symptom is phantom smells. I will smell cigarette smoke when it's not there. It's super annoying but harmless.

My 1st one was around the age of 23 and have had about 20 since then (about once/year). Triggers seem to be stress and possibly dehydration. At least two of them happened the day after losing a lot of sweat during league basketball night games
The initial symptom is usually peripheral vision getting blurry. If I take some OTC Migraine medication immediately and lay down for 30 minutes, it’s usually bearable. No splitting headaches but have the dull headache in back of head for 24 hours after.
I’m probably lucky mine aren’t worse.

Has it ever been suggested for you to see a ophthalmologist or more specifically a neuro-opthalmologist.
While certainly can just be migraine induced there are some other more primary ocular conditions that can give you similar symptoms.

I could have written your first paragraph. I bet you have the damn post covid headaches too. Per my doctor I tried the non flushing niacin 2 pills a day and it has been working

I was diagnosed w/cluster headaches and have had them off and on wince 2003. However, 2008 was probably the worst case I've had. Was actually admitted for a few hours, but was released and given a script for meds. By the time we drove to the pharmacy, 5min drive, my headache was back. Best and only thing that has ever worked is imitrex(?). I use an auto injection and usually knocks it out within a couple minutes.
