Match play rules question

Multiple day team match play tournament. Five matches total
Announced before every round that 14 club maximum will be enforced. Rules package handed to every team prior to first match had it listed.
On day three after match 4, player is determined to have 15 clubs.
Player admits that he has had all the clubs in his bag since start of tournament because a club is “broken”
His team is penalized two points for match number 4 only.
Questions
Should his team been penalized for each match?
Should the points he forfeited be given to the teams that he played against?
