Mixture of fish (mainly tuna), Vegetables (usually asparagus and avocado), soy sauce, and sesame oil. There can be other ingredients added such as seaweed, squid, onion, sesame seeds, etc...
Very good, I like the one at Kyoto in New Orleans the best. Sorry no help for BR but I assume most sushi places will have an offering.

Oh... Sounds like the Hawaiian seafood salad at Sushiyama on Perkins near Staring. Pretty delicious actually.

Description from their menu: Mango, asparagus, avocado, tomato, crabstick, and assorted fish with cilantro, lime juice, sea salt, white truffle oil, sriracha, sesame seed, and smelt roe

OP: order the tuna three ways at rock n Sake. basically three takes on a poke bowl. it's off menu
