It’s possible that it’s the shower screen above the portafilter that is partially blocked, causing the pressurized water to spray unevenly. Perhaps remove and clean the shower screen. At least backflush with cafiza.

Will a descaling agent solve this issue as well or do I need to buy Cafiza separately?

Shower screen seems most likely culprit.
