Patio Extension

I want to add a small 15'x11' concrete extension to my existing patio. I plan on forming it up myself. My question is does anyone have a recommendation for where to source the concrete from (bagged concrete, truck drop off) and someone to pour and finish the concrete. I need about 1.5 yards for a 4 inch thick slab.

Whoops, I did a calculation with only a 10'x11'. It's a little more than 2 yards.

Might be hard to get a truck to drop off that little amount without a delivery charge. A friend and I each did an extension around the same time and it was a little over 5yds^3 total. The company would've charged an extra $200 for anything less than 5yd3^3. They were cool with mixing that much and delivering to two locations that were within a few miles of each other and the plant at the normal per yard price. We helped each other form the previous weekend and had it ready for the same pour day. My buddy had help waiting at his house to finish it, and so did I. He had the truck at 9am and it showed up to mine at 11am. The driver actually helped us too
All you really need to finish it is rubber boots, a rake, and a long 2x4. I rented buggy for $65, a bull float and brush for $20 and bought an edge trowel from Lowes for $10. It really isn't that hard if just doing a small amount.

Did you put a gravel layer down before adding the concrete?

No. I used a sod cutter to get to dirt and was at about 6" down from existing finished concrete grade. I used a sandy top soil mixture from Lowes to get to about 4" below existing concrete in the center and made a 6"x6" footing around the edges. Also, I did the slab to park my truck on, not as a patio.
I think it was 20'x12' so I split it into 4 areas with expansion joints too.
