I am not familiar with Carolina Carports, but I am with Mallett Buildings. First thing I would do is check the parish permit requirements for wind rating and etc. Through my experience I would pour a 4" 4000 PSI slab, with fiber, with a rebar reinforced footing with keyway for expansion.
I had to repour my slab at my own expense after Mallett poured it. The surface finish was absolutely terrible. Core samples revealed that spots were only 2 1/2" thick, and had honeycomb around the edges. Ida damage revealed that the wing of my #1 Mallett Building was put together with nails instead of screws.
I wouldn't recommend Mallett to build you a dog house. I would fear for your dogs safety!!!
