This is more of a cultural issue and not just a sexually promiscuous issue.

How many people in a porn stars circle actually "constantly shame them".

I'd bet its the lifestyle of drugs, fake friends, being plowed and used like a cumrag on film for $1200 that make them feel useless unless they are face down and arse up. That has to take a toll.
