Looks like an older elevator, as mentioned in the article, with one of those self closing gates. Plus, with the switch she hit, she got caught between the wall and elevator when putting the package in and it started going down.

I’m not sure exactly but my guess is it was the wall by the door and the elevator. I’m guessing the large package kept the door gate switch in the “close” position, while the door was still open, and she was in the doorway thereby crushing her.

Seems crazy it’s just excused like this, but it’s possible there was a safety switch she bypassed thereby making it more dangerous then normal?
