the pilot or person in charge of a vessel underway is referred to as captain as a matter of course and courtesy in maritime tradition. Typically this only comes up in an official radio transmission or in a written account of an incident

Correct, I have no issue with referring to the operator of small fishing vessels as "captain," as a matter of courtesy, when that individual is actively engaged in the navigation of said vessel. I just find it hilarious when they continue to refer to themselves in such capacity in their every day lives.
