So basically NTSB will chock this up to pilot error.

From what is known to me it seems the pilot made several poor decisions.

Such an avoidable tragedy.

Not judging my any means but most accidents involving aircraft are caused by human error.

Approximately 80 percent of airplane accidents are due to human error (pilots, air traffic controllers, mechanics, etc.) and 20 percent are due to machine (equipment) failures.
