So we just told the hospital and collections agency we were done and haven’t heard from them in a year. I’m sure it will bite me in the arse some day.

Some should some shouldn't. The OP's definitely should not! When Reagan was in office one of the reforms was that doctors, hospitals and clinics had to work out reasonable payment schedules with the patient. As to once it goes into collections nothing can be done is bull shite. The laws state that you can make payments to the original debtor. Now since the it did go into collections call the collection company and make an settlement offer. They will probably accept. Just have comfort knowing the original debtor will get paid less than if you had continued to pay them and you get your wife's bunion surgery at a discount.

Back when I worked in finance many many years ago, medical collections were exempted from our formula for loan approvals, and it didn't hit your credit score as bad as a repo or charged off credit card. I don't know if that's still the case, it's been a while.

Medical debt shouldn't exist.

If it isn't serious you can pay an agreed amount up front. If it's a life threatening emergency you're getting billed for something you didn't agree to (like an out of network doctor who walked into the room when you were unconscious).
