Actually, the big killer was "essential" coverage. I have the data bookmarked at home. Premiums skyrocketed at the end of 2014. Community rating and guaranteed issue kicked in a lot earlier and generated modest increases.
Makes sense because "essential" coverage made insurance illegal and forced people out of affordable plans into expensive plans that were more like prepaid health care.
