Going to be one of the following.
1. The doc was having an affair, and the girlfriend's husband/boyfriend or the doc's wife had him taken out.
2. The doc's wife was having an affair and she wanted him out of the way or she just wanted him gone and for financial reasons having him killed was the best way.
3. Business or money related, the doc had financial problems and got involved with some bad people, or one of his partners had money issues and was carrying life insurance on the doc, pretty common in partnerships.
4. Something related to a former patient.
5. George Bush senior told him things he was not supposed to know, and the deep state took him out.
