He doesn't have to know about the affair beforehand. It was widely known that Cohen was Trump's personal lawyer. Someone who'd had a personal relationship with Trump would also know that. It's likely she approached him with the information and her demands. At that point, he had two options: i) tell Trump and force him to either approve a payment knowing it violated FEC laws or tell her no and have the story go public; or ii) not tell Trump, pay her off and protect Trump from the scrutiny of the affair going public and any FEC violation.
