As a college trainer, we were always told to err on the side of caution and refer for an X-Ray (and we had one in site anyway).

That said, if it's a tiny hairline fracture it can be missed. You don't miss a fractured fibula to that degree though.

sure you do...syndesmotic injury, athlete complains of ankle pain, ankle films negative. Nobody bothers to order tip/fib films to see fibular neck fx. I see it missed by physicians and mid levels all the time...
