I agree and while this may be true for those who have commercial insurance and choose to go along with all that without verifying its medical necessity, I don't. When you are a cash paying, fee-for-service individual, they don't make you do a bunch of crap just so they can bill you for it. I ask the cost and price compare everything I can before I buy it, ESPECIALLY when it comes to my health care. For example, a prescription cost can vary widely between 2 pharmacies that are right next door to one another. Getting a cavity filled can be vastly more expensive at Dentist X than it is at Dentist Y.
