Depends on what healthcare system you're practicing in. Most large ones track patient satisfaction with surveys (like Press-Ganey). So if you tell an obvious pain seeking patient that you're going to send them to physical therapy and not give meds, it obviously hurts your scores.
This isn't the same as decreased reimbursement to the hospital/clinic (like a "quality measure"/readmission rate within 30 days for inpatient), i.e. Lowered Medicare reimbursement rates, which is What I think you are talking about.
