No underwear. You'll undress with them out of room and cover with a towel. They will keep you covered appropriately throughout the massage. That's the norm.

However, if you aren't comfortable with that, leave your underwear on. I'm sure they don't care either way but most underwear will be in the way a little for a full body massage.
