I may have missed it, didn't he tell her to stand down or something when she first came in?

She didn't listen, and I know it's easy for us to 2nd guess, but given he has more info than her, you'd think he would have been more vocal to tell her to stand down.

Even still, I wouldn't place too much blame on him, even if he wasn't more vocal, I don't think he should have assumed she would do what she did given that it seemed to be under control.
