Drinks Bourbon with no mixers and no ice
Smokes a cigar that isn't bought at a gas station and doesn't have a flavor other than the tobacco.
Opens the door for people
Shines his shoes every morning

ON #1 I know several gentlemen (me included) that use a mix. I have to or not drink I have an ulcer and throat issue that if I don't makes my life hell.
Cigars - agree
Opens the door? Yep always for everyone not just women
Shines shoes? Yes. Every morning? No one does that.
Belt? not always. Certain suit combos call for no belt
Cargo shorts? A true gentleman doesn't even know what those are.
Job - yes because a true gentlemen knows integrity and work are connected
