They call it all of the time. Go read the rule about it.

It applies all game, but you only get a few throw + the ball in the last 2 minutes. I already posted that in this thread. It doesn't get called much in the last 2 minutes because people foul the ball handler because they know the rules.

I can't think of ever seeing it called. Like ever... In 25years of watching the nba.
