Nope, Bernard has been the starter since week 2.
Hill got benched for fumbles and never recovered the starting job.
Edit: I've followed the situation closely, because Hill was my first rounder in two fantasy leagues.
ETA: I just went and checked. Hill got the first touch in 12 of 13 games. No way to tell who was lined up at RB for the first play though.

I remember we had a game in 2009 where Ingram was officially recorded as the starting quarterback because our first play was from wildcat.

This is a good question.
Hill was the starter put of camp, and Lewis never publicly said Bernard was the starter. But Bernard gets double the snaps Hill does, and is usually on the field before Hill.
Maybe it goes by the official team chart, which Lewis never changed.
