Not necessarily. I was just curious whether the SSD simply died or started throwing errors beforehand. In theory, premature failure in SSDs would show no warning signs. It would simply be alive one moment and dead the next. Sounds like that's what happened. What brand/model was it?

Is it connected via HDMI? I have the same problem, but only with my second monitor, which is connected to an HDMI port on my video card using a DVI/HDMI adapter (primary monitor is connected to DVI and has no issues). Every time I boot (which isn't often), I have to turn off my second monitor and turn it back on for it to be detected.

It's obviously a Windows 10 issue, just haven't really looked into why it's happening and what specific condition is required to reproduce it (i.e., is it only for HDMI-connected monitors, is it because of the adapter, only for secondary monitors, only for SLI setups, only for Nvidia cards, etc.)
