I used to work with a guy that clearly never brushed his teeth. A green film had formed on his teeth and his breath was unbearable. No one could get enough nerve to ask what was up.
The weirdest thing was that the guy was brilliant and really did a good job. Had a wife and kids that seemed perfectly normal. He obviously wasn't popular in the office, but wasn't a complete outcast either.
Christmas Party one year, everyone is getting wasted and he is social drinking. Im talking to him and the receptionist comes up and basically makes a scene about his bad breath and green teeth. Tells him that he needs to ask Santa for teeth whitening, etc. Really embarrassed him. He put is beer down and just left without saying a word.
Comes in the following Monday like nothing has happened. The receptionist almost loses her job over it and is apologizing and the guy is really cool about it. Went to HR and asked them not to fire the woman and everything.
Guy still works there and still hasn't brushed his teeth. An old colleague of mine told me that he wins Employee of the Month once a year and is pretty much the model employee, but probably couldn't ever get another job or be promoted.

Green teeth used to be caused by an antibiotic..maybe tetracycline. It was a not uncommon happening. It also ridged the teeth a little so that they were not only green but looked dirty as well. One of my cousins had this happen. She brushed numerous times a day etc. When she got older she had them capped (or whatever). Always felt sorry for her, she could not help it. But that wouldnt explain the bad breath of this guy.
