Every time the bell rings, he looks up and you experience Tony's POV for a few moments - it definitely generated a sense of paranoia - and what it feels like to be Tony Soprano. Chase has backed off of it, but that last one is supposed to represent Members Only guy coming out of the bathroom and whacking Tony in front of his family.
In any event, my initial take was that it was ambiguous - that it is never going to get easy for Tony Soprano (even if you reject the obvious interpretation that was the moment of his death - and I've wavered back and forth on that) - and that he's going to be looking over his shoulder whether he lives 1 more second or 50 more years.
