Mostly because ERs and hospitals in general are getting busier by the day. Employers are reluctant to hire the adequate staff to compensate for this. In addition ER MDs order much more tests (xrays, CT) nowadays because they are afraid of getting sued for missing something. So even though drs know a test will be negative they order it anyway for legal reasons which puts a huge strain on the staff
