PvP in these games has always been kind of like that. I did some in ds2 yesterday and they do the same; start whipping their arse and they'll run off and heal or hide behind enemies. The biggest problem with Bloodborne pvp is that's it's a heal-fest.

I'm on team heal. After refusing to heal at first, I've realized that its an important part of PvP. Duels last 20 seconds if nobody heals. The healing animation can be parried and you have numbing mist to counter it. It adds to the strategy IMO instead of button mashing for 20 seconds. When they are about to heal and be vulnerable as they back off but they know that you know this so they try to go on the offensive instead makes things interesting. It's an important mechanic that's meant to be used. The real problem is the health drop that invaders get. Now that's an issue.
Also, I still haven't encountered anyone being a dick. Every single person has gestured to me before attacking. It's probably bc I'm the invader 80% of the time and I gesture first but still.
