I would not consider it a living world in the same manner as GTA or an actual RPG. The orc patrols are randomized each time you die, but their population is so dense across the map that it's really hard to tell. You are hardly ever at a location where an orc or patrol is not within bow & arrow range. If you took a slow stroll across the map, you would never make it b/c you would be fighting the whole time. Rooftops, bushes, and sprinting are the only way to really avoid conflict.
Now the captain strongholds actually do seem alive. Lots of orcs doing alot of different activities. Fighting, slaving, eating by the fire, training beasts, executing slaves, and forming groups to attack other nearby orc captains. And if you're seen in a stronghold, all hell is breaking loose, and you better be running away (at least this early in the game).
The thing that sticks out to me most is the enemy density. They are EVERYWHERE! It's hard to "stay on task", so to speak.
