The first obvious question:
This ENT doctor that accompanied Rivers and performed this surgical procedure: did he have privileges at this clinic?
I cannot just walk in to a clinic and start treating patients especially if I have to use their surgical equipment that I am not familiar with...
That is malpractice for both the doctor and the clinic.
