Never fallen, but I've jumped plenty of times. We were sailing around the bay at Panama City a couple summers ago, and coming up on a beach area. Boat was going too fast, and there were people around, and I had to pee. Just dove in so I didn't have to hang it off the side and scare people at the sight.
And leaving the same beach, our anchor line got tangled in another boats way too long anchor line, and we were being yanked around and about to hit another boat. Dove in, freed up the line, and pushed off the other boat so we didn't hit it.
I did see a guy take a spill off of a deep sea charter one time though. He was climbing around the side to get up front and sit on the bow for a ride to the next fishing spot, and his hand slipped on the railing and he fell in. We had to go back and pick him up. He's lucky he fell the way he did and didn't get caught up in the side of the boat.
