It seems really difficult, but it's not as hard as it seems to decide to swing in time.
You pretty much decide right when you see it leave the hands. You can tell where it's going.

I am friends with a guy who was a pitcher in the Major leagues for number of years (John Thomson)...when he was in the minors I asked if I could just stand in the box and have him throw a fast ball and let me see what it looks like. he threw like 93-94. I could not see the ball.
