I had the same thing happen to me this year, not the fire on a second trigger pull because I ejected the bullet an there was a tiny mark on the primer. I figured it was the firing pin but my gunsmith said the lock on the bolt was partially locked. You know the c shaped keyhole on the back of the bolt. I had never locked or even fooled with it at all. He said with a partial locking it could allow the trigger to be pulled but with not enough gusto to shoot the round. Maybe it's that??

I have a Tundra and researched this before I bought. As far as I saw all the accidents were caused by aftermarket floor mats that the accelerator got stuck under.
