Good list -- I'm female so some don't apply to me but I go to a cardiac prevention center in Lafayette -- she does the majority of that list twice a year. Somehow she has a deal with her labs that the cost ends up being $150 or so (she uses two different labs).
With my family history and that nominal cost, I'm good with it. I wills ay she has $500 per year "fee" but my visits are ~$100 and I've never paid for the carotid ultrasounds either (maybe once per year?).
AND the bonus for me is she'll call in meds if I'm sick with easy things .. sinus infection, uti, etc.
