When they announced that he was having surgery for an "enlarged prostate," I knew this was coming. I don't think they operate if the prostate is merely enlarged.

That's why they call it "benign prostatic hypertrophy" (BPH) . But, sometimes surgery helps. Not sure when you call it. Unable to pee, maybe?

They will if it's cutting off urine flow and medication no longer works.

Actually it has become quite common to do a prostatectomy for Benign Prostatic Hyperplasia or enlarged prostate. It is also not unusual to find foci of carcinoma in the enlarged prostate once it has been removed. According to the grade and extent of the carcinoma, further treatment may be required. A low grade carcinoma in a 75 year old man, likely will just bear watching it to see if it progresses. You can do this by comparing the PSA pre and post surgery.
