This is really the biggest question. 10 lbs over a few months isn’t a ton of weight loss, especially if he has some weight to lose. Is he actually not eating or not eating as much as he use to? Not eating at all would likely result in more than 10 lbs lost over several months.

He still eats, but very little protein and nibbles on shite carbs
