The work environment goes all the way back to how the administrators/management employees orchestrate what goes on. If any of the higher-ups are incompetent, it trickles down to the staff RNs and creates a dysfunctional environment. The result is that the quality of care is lower and everyone is pissed at each other.

Hospitals need to weed out low quality nurses for ones who perform better.
Why?
Because payment to them is issued based on treatment, not quality.
