My OB called me once when I was expecting. Like it was actually him and not a staff member. He said “Hey cubbies, it’s Paul.” I didn’t even know his first name at the time he has the best bedside manner.

2/3 of my professors this semester introduced themselves with their first names and said we can call them by their first names or Dr. _____. Whatever is fine. I still use Dr out of respect since it’s an academic setting. And I never called my OB Paul either.
