So the main reason people are obese is not diet related but hormone related? If they don’t change their diet and get on this pill will they lose weight?

Well no. Being a GLP-1 agonist, in high enough doses it reduces appetite and tricks the brain into feeling like the stomach is "full". Most people seem to change their diet on the meds, but they can always revert if/when they go off it. It is just like any weight loss regimen once you reach your target weight you still have to maintain a proper balance of calories in/calories out to avoid gaining weight.
