I blame pull-ups/chin ups for mine. I was doing multiple sets of those at the time and think I overworked the tendon. I have quit doing those entirely. I can sometimes feel a slight twinge in my arm when doing lat pull downs, and I immediately reduce the weight. Oddly, none of the bicep curl or other pull exercises give me problems. But I’m still concerned about it now, 3 years later.

Mine was either weighted chins or hammer rows. Mine hurts most on exercises where palms are supinated like chin-ups and curls. Got to the point where picking up my little boy was excruciating.
