I have done a lot of machine guarding cases in my career and probably 50% of the injuries or deaths were a direct result of modified interlocked guards to speed production time.

So many times someone thinks safe is "too safe" and endeavors to "fix" the problem in the name of a dollar. Burning the guilty party to the ground financially is "good" but it will never bring someone back from the dead or regrow their arm.
