Tell them you want to schedule a limited exam with 1 PA to diagnose a broken molar you would like fixed. Tell them you only want to be treatment planned for the single issue. Tell them you are in pain and only want a resolution for that pain. You have no interest in seeing or speaking to a hygienist.

That should keep most of the upselling at bay. Pain is the magic word at my practice. If a patient is in pain, staff is never to present cosmetics.
