I had a huge bout with toe hits, the main cause and definitely my issue was a steep downswing. Getting shallower fixes so many poor outcomes and why almost all elite ballstrikers are in a downswing plane flatter relative to the backswing.
Now what caused this for me was rolling my left arm open on the backswing. Keep that left elbow pointing at the target/down. A great analogy is to imagine you were swinging an axe at a tree. That lead elbow will be point at the tree.
Video yourself with a cellphone, if your hands are higher at impact that they were address this will be the culprit, its how you rescue an overly steep doenswing.
