With HIFU and radiation tissue could be damaged that would make nerve sparing surgery much more difficult and the only way to truly know if they got it all out is with surgery. When I was diagnosed 6 years ago HIFU was available and is still very new. I asked of the 3 options at the time of my diagnosis of surgery, radiation or active surveillance which gave me the greatest 10 year chance of survival and was told with Gleason 6 the were all the sam. I chose active surveillance which meant quarterly PSA’s and a couple of MRI’s over a 5 year period, when it showed progression I chose surgery because I wanted it all out and the entire prostate not just the cores could be examined. A few things I’ve learned is to trust but verify and leave no stone unturned you are have to be your best advocate.
