I am going to go with about 90% of the Devil's Rejects. I only saw it once a while ago and my memory does not retain much in the way of details but the tormenting of the couples in the motel room probably evoked the closest thing to pure vigilante hatred that I have ever experienced.

Yeah...that bothered the heck out of me. Was watching with my wife and her friend, but I walked out at that point.

the worst is you get a sense that Rob Zombie thought we were supposed to be cheering for them too

The knife scene in SPR is immediately what I thought of too. But that entire movie is just tough to watch. The opening scene, when Vin Diesel gets shot but they are pinned down by the sniper and no one can help him, when the medic gets shot and they don't know what to do.

A TV scene that comes to mind is Oberyn Martell getting his head smashed in

You saw a really bad version if that was the opening scene. You missed a few critical pieces.

I will go with AHX curbing and the chainsaw scene from Scarface.

Honorable mention goes to the scene from The Beast where the Afghan dude is executed by being run over feet first by a tank.

Not a movie, but TV scene...

The stoning scene in Leftovers Season 1 was the first thing that came to mind, I almost had to turn it off.

I have listened to the real guy's account of breaking and cutting off his own arm, and he said breaking his two bones was less painful than trying to cut through that nerve. He said by the time he got to the nerve, the knife was about as dull as a butter knife.

When Tom Hanks knocks his tooth out with an ice skate in "Castaway". Tough to watch, and I'm a dentist!
